SUPPORT AVAILABLE TO MAINTAINED SETTINGS IN TOWER HAMLETS

If you still require additional support please complete this referral form and send to: Catherine McNerney, REFERRALS, Phoenix Outreach Service, Phoenix School, 49 Bow Road, London, E3 2AD.

Please note: You must also include a copy of evidence that the child has a diagnosis of ASD (ASDAS or CAMHS report or equivalent). As a setting you will be expected to set a date for a TAC meeting following the referral.

When the referral form is received, a member of the outreach team will get in touch with you. You will be expected to organise a TAC and invite any involved professionals to attend. The outreach teacher will visit school once following this meeting, after which the case will be ‘on hold’ on the caseload. Subsequent referrals for support can be made after one term. If a statement or EHC plan is received (after referral has been made or while case is ‘on hold’), the child will automatically move to the standard package of support at Level 1.

Support for PVI (private voluntary independent) & non-maintained settings

It is unusual for a Phoenix Outreach teacher to visit a non-maintained setting. If you have concerns about including a child with autism in a non-maintained setting, please consider the following:
